Umbrella Multicultural Health Cooperative is a member-driven, not-for-profit, community health centre (CHC). Our members are people who self-identify as facing language and cultural barriers accessing health care. Our mission is to provide practical access to affordable and holistic health care services that are appropriate in culture and language for those without access to health security. We work with a Cross Cultural Health Broker (CCHB) Model. CCHBs are multicultural, multilingual health care workers who facilitate communication between patients, service providers and health and social service systems. Umbrella Co-op CHC is located in Downtown New Westminster. NP role at Umbrella Co-op Working with an interdisciplinary team, with an understanding and commitment to the community health centre and CCHB model, the Nurse Practitioner will provide specialized comprehensive and longitudinal primary care to attached patients from self-identified populations of racialized im/migrants, refugees and asylum seekers that Umbrella Co-op CHC serves. In addition to the Nurse Practitioner, the interdisciplinary team of the Clinics+ program is currently comprised of CCHBs, family physicians, a registered nurse, pediatricians, MOAs, social determinants of health (SDH) coordinator, a social worker, a clinical counsellor, administrative staff and volunteers. The program operates in-office and virtually Monday to Friday, and occasionally with the Mobile Clinic program which operates on Sundays throughout the Fraser Valley and virtually. The Nurse Practitioner is responsible and accountable for the comprehensive assessment of patients, families and communities including diagnosing diseases, disorders, and conditions. The Nurse Practitioner initiates treatment including health care management, and therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits, and conditions. The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary team-based health care team, making referrals to specialist physicians, allied health care and SDH resources as appropriate. This is a permanent position of 17-20 hours per week, and may include weekend and evening work. New graduates are welcome to apply, and mentorship and training will be provided. Hourly wage range is $72-85/hr, commensurate with experience and aligned with Health Authority rates and subject to related increases. The position is eligible for extended health and dental benefits, a health spending account and a group retirement plan. Paid vacation/ sick leave starts at four weeks. The position is also eligible for reimbursement of college fees through NNPBC, and access to funding for professional development. The role additionally has access to PCN opportunities, including opportunities to collaborate with Fraser Northwest Committees and/or quality improvement projects with compensation through NNPBC. Qualifications · Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM). · Recent relevant clinical nursing experience. · Demonstrated understanding of social determinants of health and barriers faced by racialized ethnocultural communities. · Experience working in a culturally diverse, community-based setting and in team-based care. · Fluency in one of the languages of the communities we serve is an asset. *Start date*: Late July or August 2024.
