_*Make a Difference in a New Generation of Long-Term Care*_ Glen Hill Gardens is a brand-new, modern long-term care home in Bowmanville, operated by Durham Christian Homes, a not-for-profit organization with a longstanding commitment to providing exceptional care and services to seniors. We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our team in a full-time position. This is an exciting opportunity to practice in a newly built long-term care environment and play an important role in shaping resident care at Glen Hill Gardens. As a key member of our interdisciplinary team, the Nurse Practitioner will have the opportunity to build meaningful relationships with residents and families, exercise a high degree of clinical autonomy, and provide leadership and mentorship to our nursing and care teams. *About the Role* Reporting to the Administrator, and accountable to the home's Medical Director in relation to medical services, the Nurse Practitioner functions as a primary care provider within their legislated scope of practice. The NP works collaboratively with residents, families, physicians, nursing leadership and the broader interdisciplinary team to assess, diagnose and manage residents' health needs and develop individualized plans of care. The position also provides clinical leadership, consultation and mentorship to staff, supporting our ability to provide high-quality care to residents within their home. *What You'll Do* * Provide comprehensive primary care to residents as an integral member of the interdisciplinary health care team. * Complete comprehensive, person-centred health assessments on admission, annually and as clinically required, including participation in medication reviews. * Obtain comprehensive health histories and assess residents' physical and psychosocial health needs. * Diagnose and manage acute and chronic health conditions within the Nurse Practitioner scope of practice. * Develop, implement and evaluate individualized plans of care in collaboration with residents, families and the interdisciplinary team. * Prescribe and manage pharmacological and other therapeutic interventions and perform procedures within the NP scope of practice. * Order and interpret appropriate screening and diagnostic investigations and ensure appropriate clinical follow-up. * Engage residents and families in meaningful discussions about health goals, care preferences and quality of life. * Provide education and guidance to residents and families and participate in resident care conferences. * Collaborate and consult with physicians, nursing leadership, registered nursing staff, allied health professionals and external health care providers. * Provide clinical consultation, support and mentorship to nursing and interdisciplinary team members, particularly in complex or challenging clinical situations. * Make and receive referrals and coordinate access to specialists, services and other health care providers. * Support transitions of care, including collaborating with hospital discharge teams to facilitate residents' safe return to Glen Hill Gardens. * Advocate for and provide palliative and end-of-life care that respects resident goals and preferences. * Maintain timely, accurate and comprehensive clinical documentation. * Contribute to education, quality improvement and evidence-informed practice initiatives that enhance resident outcomes and strengthen clinical practice throughout the home. *What You Bring* * Current registration with the College of Nurses of Ontario in the Extended Class. * Demonstrated knowledge and understanding of primary health care and the unique clinical needs of residents living in long-term care. * Strong clinical assessment, diagnostic and decision-making skills. * A collaborative approach and the ability to build effective relationships with residents, families, physicians and interdisciplinary team members. * Strong communication, leadership, consultation a- Location Bowmanville, ON
